Custodianship Declaration (IMM 5646)
IRCC uses form IMM 5646, Custodianship Declaration, for minor students. It is normally prepared in two parts: one signed by the parents or legal guardians abroad, and one signed by the custodian in Canada. Both are signed before a notary.

Who needs to attend
The custodian in Canada signs in person here. The parents' copy is usually notarized in their own country. Always follow the current instructions from the school and IRCC.
Common reminders
Complete the form in advance but do not sign it, and check that the student's name and date of birth match the passport exactly to avoid having to redo the declaration.

Please complete the form in advance, but DO NOT sign it. Bring valid government-issued photo ID and visit our office at 20 Bamburgh Cir (no appointment needed) to sign and have it stamped in front of a notary.
